Referring Patients to QUIT LINE

Referring your patients to OhioQuits is easy. And it takes less in-office time than the traditional 5 A’s method.

You do 3 A’s and an R. OhioQuits does the rest.

Ask if patient uses tobacco.

Advise patient to quit, personalizing the impact of tobacco use.

Assess willingness to quit by asking:

“Do you intend to quit within the next 30 days, or have you recently quit?”
“Would you like support through the quitting process?”

Refer patients who answer “Yes” to OhioQuits, by faxing a signed Fax Referral Form to

800-646-1103.

An OhioQuits specialist will do the rest (Assist and Arrange). Your patient will be called within 24 to 48 hours of receipt of the Fax Referral Form, when it’s comfortable and convenient for them.

If your patient is not ready to quit, provide them with a patient brochure (included in your free Quit Kit). Then, when your patient is ready, they will have all the information they need to call 800-QUIT-NOW.
